Printable repair ticket and work order template.

A repair ticket does three jobs: it records what came in, it states what the customer agreed to pay, and it tells the bench what to do. This free one-page work order does all three for phone, tablet, computer and console repairs, with a tear-off claim stub for the customer. Print it as is, or edit the terms for your shop.

  • Header: your shop name, phone and address (blank for you to fill or stamp), ticket number, date in, promised date.
  • Customer: name, phone, optional email.
  • Device: type (phone, tablet, laptop, desktop, console, other), brand and model, color, serial number or IMEI, accessories received, who handles the passcode.
  • Problem and condition: what the customer reports, and the condition on arrival (cracks, dents, liquid, missing parts). Use the intake checklist if you want the full test list.
  • Estimate: repair or part, part quality, quantity, parts, labor, line total, then subtotal, tax, total, deposit and balance due at pick-up. A line for the extra amount the customer approves without a call.
  • Signatures: customer at drop-off and at pick-up.
  • Bench section: assigned tech, status, date completed, repair performed, parts used from stock, and a tested-after-repair row (display/touch, cameras, audio, charging, buttons, signal/Wi-Fi).
  • Claim stub: ticket number, device, total quoted, deposit, ready-by date.

Terms (edit them)

Repairs can erase data; back up first. To the extent the law allows, we are not responsible for data loss or for faults not listed above that existed before the repair. Liquid-damaged devices may fail at any time. Warranty covers the parts we installed and our workmanship for ___ days, per our warranty policy; it does not cover new damage. Devices not picked up within ___ days of our “ready” notice may be treated as abandoned as allowed by local law.
Tips

Tips that save arguments

  • Write the part quality on the ticket. “Screen, $89” and “Aftermarket screen, $89” are different promises.
  • Fill the condition box before the device leaves the counter. A photo helps, but the signed line is what customers remember.
  • Keep the terms short and local. Abandoned-device rules and warranty minimums depend on where you operate; check yours. This template is not legal advice.
  • Number tickets in order so a missing ticket stands out.
